Chronic diseases like diabetes, hypertension, COPD, and heart disease are among the leading drivers of poor health outcomes and rising healthcare costs, but with the right ongoing management, many complications are preventable. MCM ACO’s Chronic Disease Management program provides the structure, monitoring, and support patients need to keep these conditions in check.

Our approach starts with identifying patients living with chronic conditions and stratifying them by risk level, so that those who need the most attention receive it first. From there, our clinical teams work alongside primary care providers to implement evidence-based management protocols tailored to each condition, blood sugar monitoring and education for diabetic patients, blood pressure tracking for hypertension, pulmonary function monitoring for COPD, and cardiac risk management for heart disease patients.

Consistency is critical to managing chronic illness, so our program emphasizes regular check-ins, medication adherence support, and patient education. Many complications from chronic disease arise not from a lack of available treatment, but from gaps in follow-through, missed medications, skipped appointments, or a lack of understanding about warning signs. We work to close these gaps through proactive outreach and patient engagement.

We also help physicians track patient progress over time using data and reporting tools, making it easier to spot when a patient’s condition is trending in the wrong direction and intervene before it results in an emergency room visit or hospital admission.
